Meeting Room Request Form

NOTE: All meetings must be concluded and building vacated by 4:30 p.m.  We have no custodial service; therefore, meetings will not be scheduled in the evening.  It is the responsibility of the requesting organization to arrange the room and return it to the original set-up if any change is necessary.  Any clean up is the responsibility of the requesting organization.   There is no charge for the use of the meeting room.  We would rather not exceed 40 people in the room.
