Human Resources: Licensure Information

Substitute Teacher Information and Checklist

2010-2011

 

Each substitute must apply at the Butler County Educational Service Center to be eligible to sub at the following local school districts:

 

   Lakota**    Madison     Monroe New Miami     Ross

 

  • Either an Ohio Standard Teaching Certificate/License or a Butler County Substitute Certificate/License is required.  Expired Ohio Certificates may be validated for substitute service.  To print a copy of the substitute license application click hereYou may also call Rhonda for an application to be mailed to you. I encourage you to consider applying for a five-year sub license.  Return the application with appropriate fees and transcripts, if necessary, to our office.

 

  • New Substitutes must present evidence of a negative tuberculin test (Mantoux test preferred) performed within the last 90 days.  A chest X-ray is necessary in the event of a positive reaction.  Names will not be placed on the official list until this requirement is met.  The skin test can be secured, at your expense, at your own Family Physician or from the Rosin Health Clinic, located at 301 S. Third St. in Hamilton.  You can phone their office at 887-5253 to learn of their hours.

  • A State BCII and Federal (FBI) criminal record check (fingerprinting) is required by the Ohio Department of Education (ODE). When an individual submits an application for their very first license, certificate, or permit issued by the Ohio Department of Education, a BCII and FBI background check report completed within 365 days of the date the application is received, must be on file at ODE.  For background check information regarding the renewal of your license, see Renewals and Additional Licenses, Certificates or Permits information found on page 1 on all license applications.

 

Your fingerprint results MUST be submitted and electronically sent to the Ohio Department of Education via WebCheck for licensure purposes.  Send results of BCII and FBI to:  Butler County Educational Service Center, attention:  Rhonda Miller, 1910 Fairgrove Avenue, Suite B, Hamilton, Ohio  45011.

 

Fingerprinting can be done in our office at the Butler County ESC,1910 Fairgrove Avenue, Suite B, Hamilton, Ohio  45011 on TUESDAYS from 9:00 a.m. to 11:00 a.m. and THURSDAYS from 1:00 p.m. to 3:00 p.m. 


*Beginning Tuesday, September 28, all fingerprinting will be at our new location 400 North Erie Boulevard, Suite A, Hamilton, OH  45011.

Sign up for a SAFE account via the Ohio Department of Education Homepage through the following website:  www.ode.state.oh.us.  If you have questions regarding SAFE, please contact SAFE.ADMIN@ODE.STATE.OH.US.

The SAFE account provides a username and password to use in Educator Profile. Educator Profile provides information regarding application status, credentials, and payments received at the ODE, and BCII/FBI background information.

**Lakota has instituted a system for calling Substitutes (AESOP).  You must still follow these procedures for remaining on the approved sub list.  Information regarding the Lakota AESOP system will be forthcoming.  You can contact the Lakota Human Resources Department at (513) 644-1193 for assistance with AESOP.

 

Substitutes are called from the local district/schools and not through the Butler County Educational Service Center.

Checklist and Fees:

 

  • Substitute Teacher Application (Click the text at left to open a form that may be completed online and printed) or Ohio Certification/License Renewal (Transition) Application, filled out completely with a check or money order payable to “Treasurer, State of Ohio.”  Fees are noted on the application.
      • or a COPY of your current Certificate/License
      • Official Transcripts are required in order to approve your initial Application.  (Please note ODE does not return transcripts.)
      • Substitute Nurse Application with fees; all nurses will need to submit a copy of their current Nursing License.   

  • Copy of negative Mantoux tuberculin test - Lakota substitutes are required to submit a copy of their skin test to the Lakota Human Resources Department located at 5572 Princeton Road, Liberty Township, Ohio  45011-9726.

 

  • Copy of State and Federal background checks - must be within 365 days old.

Money order(s) only required in the amount of $22 & $24. Leave recipient blank; (Not Cash and Not Personal Checks).

  • BCII will issue upon request, a free copy of your BCII record check if requested within the first 30 days of being fingerprinted. If you have been fingerprinted within the past year, we have a form that we can send in with a money order only in the amount of $8.00.  This form will require name, birth date, social security number, and your signature. For a copy of your FBI record check, identity must be verified with a valid govSubstitute Teacher Information Formernment issued photo ID and your signature is required.

 

  • Driver’s License or State ID Number
