The link to the right entitled "Employee Forms" is being provided as a resource to the employees of the Butler County Educational Service Center. The files being provided are in PDF format and are formatted so that you can type directly on them and printout upon completion. Please note that you are not able to save your completed form. The Excel forms can be saved to your local hard drive and the information that you enter is saved as well.
